HEALTHCARE

Two half-day medical clinics per week provide safe supply and primary care services such as general assessment, wound care, bloodwork, STI testing and treatment, referrals, etc. Specialized to treat marginalized populations.

No health card required.

OUTREACH

Every other Sunday, a team of K6 peers, volunteers, medical students, and HAMSMaRT team members go out and distribute harm reduction supplies, wound care kits, clean underwear, socks, hygiene kits, and seasonal necessities (e.g. sunscreen, hand warmers).

HOUSING & OTHER SUPPORTS

Our community liaison assists with housing efforts, ODSP applications, tax filings, and connections to other city supports and services. This service is available during clinic hours.

Outreach Clinical Care

OUTREACH

Outreach nursing services are available for clients on an as-needed basis to meet clients that are temporarily unable to come to clinic. Our RN can come to homes, shelters, community centres, or encampments if needed.

In collaboration with Hamilton Paramedic Services, Positive Health Network, and Hamilton Public Health, HAMSMaRT is participating in a new program! This program strengthens Hamilton's drug poisoning response through individual client-led outreach support and service coordination.

 

The outreach team (Monday-Sunday 11am-7pm, Wednesday 12pm-5pm) will be comprised of Harm Reduction Workers and Peer Support Workers who travel within community to visit individuals who have recent experienced a drug poisoning. They will provide post-overdose support, harm reduction supplies, harm reduction education, referrals and service connection, and peer support. This is the team that HAMSMaRT will be supporting!

The service coordination team (Monday-Friday 8:30am-4:30pm) will support healthcare partners, community organizations, and the outreach team by providing a direct link to health and social services for underhoused individuals who use drugs in Hamilton. They will be supporting service navigation across the city and work to strengthen continuity of care following urgent events, such as a drug poisoning.

You can now request outreach support when an individual:

·  has recently experienced a drug poisoning

·  has declined transport after EMS response

·  has visited or is leaving hospital following a drug poisoning

You can also request service coordination when:

·  support is needed to connect a client to health or social services

·  service navigation is complex or unclear

·  coordination between multiple providers is required

MENTAL HEALTH

Drop-in Psychiatry

Drop-in psychiatry services available on a first-come first-serve basis every Tuesday and Thursday 1-4pm. No waitlist and no referral needed. 

DONATION

Clothing & Hygiene

Donated clothing, new underwear, new socks, and hygiene kits are available in clinic to anyone in need during clinic hours. Sleeping bags and tarps are also available dependent on supply.

Clothing donations are accepted! Email clinics@hamsmart.ca if you have clothing to donate.  

Naloxone

OVERDOSE RESPONSE

Injectable and intranasal naloxone available for pick up during clinic hours. Expired naloxone accepted for proper disposal. Individual or group overdose response training also available on request. Naloxone supplied from Hamilton Public Health.  

PCAAT

PRIMARY CARE PATHWAY

The Primary Care Access and Attachment Team (PCAAT) is a collaboration between CMHA, Shelter Health Network, Good Shepherd, and Refuge Newcomer Health, creating a pathway for accessible healthcare and connection to a family physician. Referrals to connect with a family physician can be completed by a member of the PCAAT team during clinic hours. 

HARM REDUCTION

Sterile injection and inhalation supplies available for anyone during clinic hours. Safer sex supplies and small sharps containers also available. Return any used equipment to be disposed of properly. No ID or name needed.

DRUG TESTING

Fentanyl and xylazine drug testing strips available for pick up during clinic hours. No ID or name needed. Testing kits supplied from Public Health Hamilton. 

POC Testing

STI TESTING

Point-of-care testing for HIV and Syphilis available during clinic hours through nursing support. Treatment plan available for all positive tests. 

Point-of-care testing for HIV, Syphilis, and Hepatitis C available twice a month from 1-3pm during clinic through a collaboration with Shelter Health's Hep C Team. 

SAFE INJECTION SITE

YWCA Safer Use Space

Our collaboration with the YWCA supports the medical directive for the Safer Use Space. Women and gender-diverse people can access the space during operational hours to access sterile injection/inhalation supplies or inject their substances on-site with trained staff on standby to respond to overdoses or adverse events. Peer support is also provided. Located at the Hamilton YWCA McNab Location.
